(i)  Certificate of Private company limited Registration in Andhra Pradesh.(ii) Experience Certificate for 2 years in Medical related IT projects. (Preference will be given to higher experience)(iii) Experience in implementation of HMIS at least for 2 years in any hospital of not less than 500 beds (Preference will be given to higher experience)(iv) Latest VAT clearance certificate with 2 years statements.(v)   Latest IT returns for 2 years.

          Sl.No.8 A.:     After completion of the contract, the Agency has to leave the premises leaving the equipment and software to this hospital.

          The 3rd para in Contractor's bid is deleted.

TENDER NOTICE

            Sealed tenders are invited from reputed service providing agencies with minimum 2 years of experience(Preference will be given to higher experience) in the medical related IT projects for computerization (electronic hospital management system )of all services and Aarogyasree documentation, in King George Hospital, Visakhapatnam.The tender schedule including specifications,terms and conditions will be issued on payment of Rupees 5000/-(five thousand rupees only)+ 14.5% vat(aggregating of Rs.5725/-Through Demand Draft in favour of Superintendent,King George Hospital,Visakhapatnam from any Nationalised banks ,and available in the office (G1 section) from 28-05-2013 to 08-06-2013, from 11 am to 4 pm on working days.
